Discipline checks, background checks and other documentation seeking to verify a students enrollment and disciplinary history can be sent to the Office of the Dean of Students for completion.
This type of documentation is typically needed for students wishing to transfer colleges or universities, may be requested by an employer, the Board of Law Examiners and/or the United States military.
Frequently these types of forms and documents may also be sent to the Office of the Registrar, in which staff from the Office of the Dean of Students will work cooperatively to get the documentation completed.
Requests must be signed by the student/individual whose information is being requested.
Dean of Students staff will work to complete forms or write supportive letters (if no form is available) as quickly as possible. To help expedite the process, please make sure the following information is provided in your request:
Your name, email address and university ID number.
Address, email or fax number where the documentation should be sent/returned to.
Please anticipate that completing documentation will likely take between 7-10 business days during the academic year and may take longer during periods of reduced staffing and operation (summer, winter break, thanksgiving break, spring break).
